About the RoleThe Care Transitions Specialist serves as a trusted clinical and community liaison across Haven’s Home Health, Home Care, and Hospice services. You will partner closely with referral sources, patients, and families to provide education, guidance, and quality‑of‑life focused consultations that support timely and appropriate care decisions.
This position blends clinical knowledge, community outreach, and relationship management, all grounded in Haven’s commitment to doing what is right, not what is easy.
What You Will Do Build and Sustain Community Relationships- Manage an assigned territory and develop strong referral partnerships with hospitals, skilled nursing facilities, assisted and independent living communities, physician offices, and other community organizations
- Represent Haven at community events, in‑services, networking opportunities, and educational sessions
- Serve as a consistent, reliable resource for referral partners
- Promote timely, appropriate referrals that support quality outcomes and patient‑centered care
- Track referral activity, consultations, outreach visits, and outcomes with accuracy and consistency
- Share market insights, community feedback and referral trends with leadership
- Work closely with interdisciplinary clinical teams across all service lines
- Help identify appropriate care pathways across all service lines and coordinate next steps with admissions and clinical teams
- Uphold all regulatory, ethical and organizational standards
- Minimum of 1 year experience in healthcare marketing, community outreach, admissions, care coordination, or a related role
- Knowledge of home health and home care services and payer sources
- Excellent communication, organization, and relationship‑building skills
- Ability to work independently while staying closely connected to a team
- Comfortable navigating sensitive conversations with empathy and professionalism
- Enjoys meeting people, building trust and serving seniors and families in need
- Valid driver’s license with ability to travel within a 60 to 80 mile radius

Salary Range: $55,000–$62,000
