Rehab and Admission Discharge Social Worker
Job Summary The acute Rehab Admissions & Discharge Social Worker is responsible for performing social work assessments, interventions as needed, and planning for patients discharge needs. They are dedicated to patient and family centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem solving.
Responsibilities- Perform social work assessments and interventions as needed.
- Plan for patients' discharge needs.
- Provide patient and family centered care, supporting personal self-determination.
- Manage multiple priorities concurrently.
- Engage in creative, compassionate, and ethical problem solving.
- Excellent understanding of rehab admission criteria (pre‑admission, admission and discharge) and insurance payors.
- Obtain prompt authorization from payors prior to patient admission to the Rehabilitation Center.
- Collaborate effectively with payors and provide timely documentation for admission authorization review.

Pay Range$39.18 - $58.28 per hour
