- Provide orientation to new patients regarding program rules, policies, procedures, and patients' rights.
- Conduct Intakes, Discharge Planning, Initial Assessments, and Treatment Plans within federal and state guidelines, CARF guidelines, and Pinnacle policies.
- Involve patients in assessment, treatment planning, and intervention to empower their recovery.
- Administer a biopsychosocial assessment covering addictions, medical and mental health history, socioeconomics, legal status, and more at intake and ongoing basis.
- Utilize patients’ Strengths, Needs, Ability, and Preferences (SNAP) to develop individualized Treatment Plans.
- Determine clinical necessity of counseling services based on assessment.
- Monitor and document changes in patient circumstances, addressing relapse indicators early.
- Comply with Medical Q.A. and Clinical Risk Management Policies, including risk evaluations and screening tools such as BAM‑R, C‑SSRS, GAD‑7, and PHQ‑9.
- Flag high‑risk patients to Medical Directors, Physician Extenders, and Regional Clinical Leadership; coordinate referrals and maintain documentation.
- Provide professional counseling and referral services, including individual and group counseling on topics such as relapse prevention, peer pressure, anger management, domestic violence, parenting, family preservation, vocational rehabilitation, etc., using techniques like CBT and Motivational Interviewing.
- Schedule regular counseling sessions, issue appointment cards, and record sessions in the EHR scheduler.
- Document counseling entries with required signatures and dates.
- Engage in community relations and outreach efforts.
- Attend all training including Counselor Physician Training, RCL training, case conferences, and required courses.
- Collect research data when requested, observe urine sample collection, attend team meetings, and complete all training courses timely.
