An exciting job role has recently become available for a dedicated and experienced Adult Social Worker to join our client’s Discharge 2 Assess team based in Oxfordshire. This is an excellent opportunity to work within a dynamic and supportive team environment, contributing positively to the lives of service users.

Responsibilities:

• Conduct comprehensive assessments of service users to determine their needs and eligibility for services.
• Develop, implement, and review care plans in collaboration with service users and their families.
• Coordinate and manage safe and timely discharges from hospital settings.
• Work collaboratively with other professionals such as doctors, nurses, and therapists to ensure a holistic approach to care.
• Advocate for the rights and needs of service users, ensuring they have access to the resources and support they require.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
• Twice weekly payroll
