Luther Street Medical Centre is a nationally recognised GP service providing high-quality healthcare to people experiencing homelessness across Oxfordshire. We’re expanding our reach and we need a dynamic, compassionate Band 6 Outreach Nurse to help us deliver excellent care directly to the communities who need it most.
It’s a chance to take your skills out into the community, hostels, day centres, temporary accommodation, and street settings bringing healthcare to people who cannot easily access it.
Funded in partnership with Homeless Oxfordshire, this role places you at the forefront of integrated health and housing support, working closely with a multi-disciplinary team including GPs, inclusion health nurse practitioners, mental health practitioners, addiction specialists, dentists, volunteers and the Oxfordshire Health & Homelessness Inclusion Team.
Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
Main duties of the job- Delivery of preventative primary care and pre-hospital health interventions to the homeless and vulnerable groups – focussing on outreach work
- Effective operation of a nursing outreach service in community settings e.g. hostels, voluntary groups, homeless day centres and temporary accommodation
- Working collaboratively with staff in community settings (e.g. supported accommodation, voluntary groups, homeless day centres) and the Oxfordshire Health and Homelessness Inclusion Team to support patient health.

- Close collaboration with and reporting to CEO for Homeless Oxfordshire will be expected as part of this role
- Run nurse-led clinics in community settings to improve availability of health services for those people experiencing homelessness (PEH) who currently do not have such access
- Work collaboratively with other relevant services to plan and implement regular outreach to reach high-need individuals
- Be proactive in delivering public health nursing interventions e.g. immunisation programmes, to the target patient cohorts
- Offer information and education on long-term conditions and other health issues relevant to PEH to frontline home lessness services to help them better support their clients
- Support hostels/day centres that do not have strong links to their local health providers to develop and enhance support…
