Responsibilities for Direct Care Worker:
- The Supervised Living Direct Support Professional will provide direct care for individuals who are designated as /DD who live in the supervised living program.
- Services provided will include:
- Maintain responsibility for monitoring and supervising individuals at all times during service provision, whether in a supervised living home, during transporting, or in the community.
- Serve as a member of the Communicare Treatment Team and participate in team meetings as required
- Perform other duties as assigned.
- Work under the supervision of the Supervised Living Supervisor or other supervisor as designated.
- Transport Clients to various activities, appointments, etc.
