Overview
We are looking for a dedicated and compassionate Registered Nurse to join our hospice team as a Case Manager
. In this critical role, you will coordinate and oversee patient care, ensuring each individual receives the comprehensive support and services they need throughout their end-of-life journey. You will collaborate closely with patients, families, and interdisciplinary healthcare providers to develop and implement personalized care plans that prioritize comfort, dignity, and quality of life.
- Coordinate and oversee patient care for hospice patients.
- Collaborate with patients, families, and interdisciplinary healthcare providers to develop personalized care plans.
- Ensure services prioritize comfort, dignity, and quality of life.
- Registered Nurse (RN) with current licensure.
- Experience in hospice, palliative care, or case management preferred.
- Demonstrated dedication and compassion in caring for patients at end-of-life.
- Ability to coordinate with multiple disciplines and implement care plans.
