Physical Therapist (PT) – PACE Program

Schedule: Full-Time, Day Shift

Salary Range: $90,000 – $115,000

About the Opportunity

RCM Health Care Services is seeking a licensed Physical Therapist for a full-time opportunity with our PACE (Program of All-Inclusive Care for the Elderly) client in Owensboro, KY.

This role is ideal for a PT who enjoys working with a geriatric population in an interdisciplinary, community-focused care model. The Physical Therapist will assess, plan, and implement therapeutic interventions to help participants achieve and maintain their highest possible level of independence and function at home and in the community.

Key Responsibilities

  • Provide high-quality physical therapy care as an active member of the interdisciplinary team (IDT).

  • Conduct in-person assessments at initial enrollment and as needed, evaluating participants’ functional abilities, home safety, and equipment needs.

  • Develop and implement individualized physical therapy treatment plans according to professional standards and participant goals.

  • Administer therapeutic programs, monitor progress, and modify interventions as necessary.

  • Educate participants, caregivers, and family members on exercise programs, mobility, safety, and home modifications.

  • Train PACE staff on safe transfer techniques, body mechanics, and use of assistive equipment.

  • Collaborate with the IDT to develop comprehensive care plans and ensure continuity of care.

  • Document assessments, treatment plans, and progress notes accurately and in compliance with all PACE and regulatory standards.

  • Participate in quality improvement initiatives, team meetings, and staff development activities.

  • Provide services across settings — including the PACE center, contracted facilities, and participants’ homes.

Qualifications

  • Advanced degree in Physical Therapy from an accredited program.

  • Active Kentucy PT license (or eligibility to obtain).

  • Minimum of 1 year of experience working with a frail or elderly population preferred; training provided if needed.

  • Experience working on an interdisciplinary care team preferred.

  • Valid driver’s license and proof of auto insurance required.
