Job Summary: The MAT Case Manager coordinates and supports patients receiving medication-assisted treatment for substance use disorders. They act as a liaison between patients, prescribers, nursing staff, and community resources to ensure continuity of care and promote long-term recovery.

Essential Functions
  • Conduct intake assessments and develop individualized treatment and recovery plans.

  • Monitor patient progress, adherence to prescribed medications, and overall treatment goals.

  • Provide counseling, education, and referrals to supportive services (housing, employment, peer support, etc.).

  • Coordinate care with physicians, nurses, therapists, and outside providers.

  • Maintain accurate, timely documentation in compliance with program and regulatory standards.

  • Advocate for patients’ needs while promoting harm reduction and recovery-oriented approaches.

  • Ensures compliance with Federal, State, and local regulations and River Valley’s mission policies, procedures and standards.

  • Completes all mandatory compliance, HIPAA and state and federal regulatory training as required.

  • Maintains an appropriate degree of confidentiality.

Qualifications

Education and Experience:

  • Requires a background in social work, counseling, psychology, or a related behavioral health field.
  • Bachelor's degree from an Accredited College in a Human Services field or a Bachelor's degree in a different discipline with two (2) years' experience working in a health care setting, behavioral health setting preferred.
  • Valid driver's license, reliable operating transportation, and proof of current auto insurance
  • Prefer experience working with children, families, and adults who experience complex emotional and behavioral difficulties which may present in crisis.

Knowledge and

Skills:

  • Knowledge of substance use disorders and evidence-based treatment approaches.
  • Strong organizational and case management skills.
  • Ability to build rapport and maintain professional boundaries with patients.
  • Familiarity with community resources and support systems.
