Overview

The Licensed Practical Nurse provides direct nursing care to residents in accordance with current federal, state, and local standards, guidelines, and regulations that govern the Community, and as required by RCC Services or the Nurse Supervisor to ensure high-quality care at all times.

The following is a general list of key duties and responsibilities. It is not exhaustive; additional duties may be assigned by an authorized supervisor.

Responsibilities
  • Comply with the Community’s personnel, safety, and corporate policies and procedures.
  • Comply with the nursing department’s policies and procedures.
  • Report witnessed or reported allegations of resident abuse/neglect immediately to the Executive Director, RCC, or Unit Manager. Treat residents, family members, visitors, and team members with dignity and respect.
  • Maintain the confidentiality of all resident and family information.
  • Report to work on time and as scheduled.
  • Assist in the admission and discharge of residents.
  • Assist residents in daily hygiene care including bathing (bed bath, shower, tub), oral, ear, eye, facial, hair, and nail care.
  • Keep resident’s clothing and linens dry by changing as needed when soiled or wet.
  • Assist residents in bowel and bladder functions; collect specimens when required.
  • Assist in transporting residents to/from appointments, activities, and social programs.
  • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Report and, when appropriate, document any change observed in a resident’s condition or behavior to the Charge Nurse. Assist in the care of dying residents; provide post-mortem care.
  • Administer professional nursing services as required (e.g., catheterization, tube feedings, suction, changing dressings/bandages, packs, colostomy, drainage bags, taking blood).
  • Perform lab tests, blood pressures, and other routine monitoring as required per the resident care plan. Admit, transfer, and discharge residents as needed.
  • Complete accident/incident reports as necessary.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, and file required recordkeeping forms/charts on admission, transfer, and discharge. Chart nurses’ notes in an informative and descriptive manner reflecting the care provided and the resident’s response.
  • Submit accident/incident reports to the Director as required; follow established procedures. Report discrepancies concerning physician’s orders, diet changes, or charting errors to the Nurse Supervisor. Perform routine charting in accordance with charting and documentation policies. Develop and maintain good working rapport with inter-departmental personnel to ensure nursing services meet resident needs. Complete required documentation of care and services as they are provided.
  • Review complaints and grievances filed by assigned personnel and make appropriate reports to the Nurse Supervisor as required. Follow Community procedures.
  • Receive/give nursing report during shift change.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per Community policies and procedures.
  • Monitor assigned personnel to ensure safety regulations are followed in equipment and supply use.
  • Recommend equipment and supply needs of the unit/shift to the Nurse Supervisor.
  • Review care plans daily to ensure appropriate care is rendered.
  • Maintain the confidentiality of all resident care information.
  • Oversee resident needs, staff, and operations as needed.

Physical Requirements:

  • Regular attendance at the worksite.
  • Must have a valid LPN license in good standing with the State.
  • Must have basic computer skills; able to read, write, speak, and understand English.
  • Must be able to work beyond normal hours and on weekends/holidays when necessary.
  • Must be able to assist in evacuations during emergencies.
  • Must be able to perform essential job functions with or without reasonable accommodation.
  • Use of transfer/gait belt or assistance from another employee when lifting or carrying objects over 25 pounds.
  • Physical requirements include lifting and carrying various weights, reaching above/at shoulder/below shoulder height, pushing/pulling, hand manipulation, and dexterity.
  • Twisting, bending, crawling, squatting, kneeling, crouching, climbing, and balancing are required.
  • Ability to relate to and work with ill, disabled, elderly, emotionally upset, or hostile individuals within the Community.
  • Ability to see and hear or use prosthetics to meet job requirements.
