Responsibilities- Provide palliative care in collaboration with the health care team to support individuals facing life-threatening illness and their families.
- Deliver holistic care addressing physiological, psychological, socio-cultural and spiritual needs.
- Coordinate with personal support workers, physicians, chaplains, therapists, hospice volunteers, and other professionals.
- Support active living and assist families during illness and bereavement.
- Current Certificate of Competence with the College of Nurses of Ontario with availability to work days, evenings and weekends.
- Minimum 1 year palliative care experience and ongoing continuing education preferred.
- Excellent communication skills, strong computer skills, and well-developed interpersonal and organizational abilities.
- Able to work independently and as part of an integrated team (PSWs, physicians, hospices).
- Effective problem-solving and conflict management skills; adaptability to changing environments and commitment to continuous quality improvement.
- Current CPR certificate is required.
