• Provide palliative care in collaboration with the health care team to support individuals facing life-threatening illness and their families.
• Deliver holistic care addressing physiological, psychological, socio-cultural and spiritual needs.
• Coordinate with personal support workers, physicians, chaplains, therapists, hospice volunteers, and other professionals.
• Support active living and assist families during illness and bereavement.

• Current Certificate of Competence with the College of Nurses of Ontario with availability to work days, evenings and weekends.
• Minimum 1 year palliative care experience and ongoing continuing education preferred.
• Excellent communication skills, strong computer skills, and well-developed interpersonal and organizational abilities.
• Able to work independently and as part of an integrated team (PSWs, physicians, hospices).
• Effective problem-solving and conflict management skills; adaptability to changing environments and commitment to continuous quality improvement.
• Current CPR certificate is required.
