Care Manager – INTEGRAcare

Reporting to Director of INTEGRAcare East, the Care Manager is mainly responsible for providing primary care management, nursing support, coordinating & scheduling care for your Clients to ensure that they maintain their dignity, independence, and joyful living.
Responsibilities- You will have the opportunity to work in one of the leading Home Care companies in Canada and work alongside some of the best Care Managers in the Industry.
- You will learn a variety of new skills related to Client and Caregiver management.
- You will have the opportunity to manage the care of our valued INTEGRAcare Clients including:
- Developing and maintaining dynamic care plans and schedules for our Clients
- Lead/coach teams of Caregivers and interact with our multidisciplinary team (Physiotherapists, massage therapists, doctors etc.)
- How to teach New Graduate PSWs various elements of caring for our Clients

- Manage your own INTEGRAcare Clients including establishing and modifying dynamic care plans and schedules
- Manage Care Teams by planning and scheduling Caregivers with a particular focus on continuity and consistency of care for your Clients
- Help teach the new hires program and assist in the orientation process of new hires
- Be part of the on-call rotation
- Participate in company and team building events
- Valid CNO Registration in good standing
- 1-2 years of Nursing experience (preferred)
- Have a great attitude and willingness to work in a fast-paced environment
- Strong Attention to Detail and desire to “Close the Loop”
- Possess good written and verbal communication skills
- Be passionate about providing our Clients and Caregivers with the highest quality care and support
- Be open to new learning opportunities, be coachable and dependable
- Have a valid driver’s license in good standing
Please note – this role will involve in-person paid training as per a mutually agreed schedule at our Toronto office at 396 Moore Avenue.
Position Details- Type of Position: Full-Time
- Annual Salary: $75,000-$77,000 + Benefits
- Location: INTEGRAcare Ottawa Office (on-site)
- Start Date: As soon as possible
