POSITION SUMMARY

The Children’s Hospital of Eastern Ontario (CHEO) Complex Care Program provides proactive and coordinated centralized care, closer to home. We promote teamwork and communication across systems of care by partnering with children and youth with medical complexities (CYMC), their families and service providers. This ensures CYMC have access to the support and services they need. Additionally, we have Complex Care satellite clinic sites to support care closer to home.

The Registered Dietitian is required to offer specialized nutrition assessments, planning and ongoing support to Complex Care patients and families. The Dietitian functions as a member of the multidisciplinary health care team and attends and participates in patient rounds and conferences with medical, nursing and allied health personnel. The dietitian is a member of the CHEO Discipline of Dietitians.

MAIN RESPONSIBILITIES
  • Provides initial and follow-up nutritional counseling to patients and families
  • Plans the nutritional care for patients on therapeutic and modified diets and parental nutrition
  • Provides specialized nutrition assessment, planning, and ongoing support for children requiring enteral nutrition, including gastrostomy (G-tube) and gastrojejunostomy (GJ-tube) feeding.
  • Participates in clinic visits in person or by telemedicine and provides direct patient care
  • Complete nutritional assessments based on diet history, medical history, and laboratory data.
  • Review patient charts and document appropriate subjective and objective data as well as nutritional assessment and plan.
  • Contributes to the Medical Single Plan of Care
  • Provide patient and family education when appropriate.
  • Evaluate the appropriateness of diet order. Prescribe and/or recommend therapeutic diets and implement nutritional care plan.
  • Interpret and process diets according to the doctor’s prescriptions.
  • Monitor adherence to and acceptance of a nutritional care plan.
  • Monitors and modifies treatment plans accordingly while evaluating outcomes.
  • Participate as a member of a multidisciplinary team. Attend and participate in medical rounds, departmental meetings, make referrals as appropriate, participate in educational programs and workshops.
