WISe CARE Coordinator

Overview

WISe Coordinator is part of a team that provides intensive wrap‑around services and support to youth and their families’ recovery needs as identified on the Children and Adolescent Needs and Strengths (CANS) assessment. The qualified candidate will ensure active participation in the implementation of Cross Systems Care plan while assuring quality mental health services that contribute to the well‑being of the citizens in Adams County.

• Ensure active participation in the implementation of Cross Systems Care plan.
• Provide on‑call response services and maintain contact with local and state social service providers and the public.
• Handle stressful and crisis situations tactfully and appropriately.
• Work evenings and weekends to meet the needs of youth and families where and when they need support.

• Bachelor’s degree in education, psychology, social work, or related field.
• Three (3) years of community or educational outreach experience, or a combination of education and experience demonstrating the required knowledge, skills, and abilities.
• High level of flexibility and accessibility, including availability for evenings and weekends.
• Excellent interpersonal and communication skills with the ability to work one‑on‑one or collaboratively as part of a team.
• Valid driver’s license.
• Successful background check and driving record check.
• Bilingual Spanish encouraged.
