OverviewThe Quality Improvement Coordinator provides nursing assistance and education to patients in an outpatient community health center, supporting a multidisciplinary care team and ensuring achievement of clinic performance measures through leadership, delegation, problem identification, and continuous process improvement. The team is committed to delivering culturally competent care to diverse socioeconomic and cultural backgrounds.
Responsibilities- Review and prioritize patient risk adjustment assessments required by private insurers and managed care organizations.
- Promote and educate care teams to manage and achieve clinical performance metrics as set by CBHA.
- Review patient test results with provider and communicate with patients to ensure effective outcomes.
- Engage in outreach events to promote and educate on preventive services.
- Review chronic care patient records and provide outreach services as required by Managed Care Organizations, patient insurers, and medical providers on a monthly basis.
- Implement plans based on assessed needs of patients with complex medical and social problems in coordination with a multidisciplinary health care team.
- Provide nursing care including patient education assessments, nursing interventions, medical case management and procedures as needed.
- Communicate with equipment vendors to ensure equipment functions according to manufacturer guidelines and safety protocols.
- Promote a positive work culture and uphold clinic policies and procedures, including the WE CARE Standards of Behavior.
- Maintain confidentiality regarding CBHA activities, reports, financials, patient health information, and other proprietary information.
- Perform other duties as assigned.
- Completion of a high school program and Medical Assistant training recognized by the State of Washington.
- One year of experience in a clinic setting.
- Ability to work under the full scope of practice outlined for a Medical Assistant—Certified under Washington state regulations.
- At least 2 years of experience in healthcare working with an electronic medical records system.
- Knowledge of proper patient lifting and moving techniques when assistance is needed.
- Familiarity with HIPAA regulations and compliance.
- Completion of continuing education requirements for licensure.
- Current and unrestricted Washington Medical Assistant’s license (MA-P, MA-R, or MA-C).
- Prepare basic correspondence using word processing and email software.
- Create tables and simple displays of information in spreadsheet software.
- Enter and manage data in a centralized electronic health record system.
- Set up and coordinate phone conferences using Web Ex and conference room systems.
- Build and maintain relationships with individuals from diverse cultures.
- Effectively meet with a variety of people, both formally and informally, and communicate clearly.
- Communicate information in written and electronic correspondence.
- Bilingual in English and Spanish for patient interactions.
