You will manage a caseload of care leavers, providing statutory support in line with legislation and guidance. This includes coordinating and completing Pathway Plans, advocating for young people, and ensuring they are supported with accommodation, education, employment, health, and emotional wellbeing.
Key Responsibilities
- Manage a caseload of eligible, relevant, former relevant, and qualifying care leavers
- Prepare, implement, and review Pathway Plans in partnership with young people and professionals
- Support young people to develop independent living skills including budgeting and life skills
- Work collaboratively with partner agencies such as housing, education, employment, health, DWP, probation, and voluntary services
- Promote young people's engagement, aspirations, and positive outcomes
- Maintain accurate and timely case records using electronic recording systems
- Participate in duty and referral work as required
- Apply anti-discriminatory and anti-oppressive practice at all times
- Contribute to service development, monitoring, and evaluation
- Substantial experience working with children and young people up to age 21 (or 25 if in education)
- Experience of working with vulnerable young people, including care leavers or those at risk of homelessness
- Experience managing or supporting a caseload
- Strong written and verbal communication skills
- Ability to build positive relationships with young people who may be challenging to engage
- Knowledge of leaving care legislation, pathways planning, and young people's entitlements
- Ability to work effectively in a fast-paced environment
- Commitment to equality, diversity, and safeguarding
- Willingness to undertake relevant training, including NVQ Level 3
- Flexible working hours required to meet service needs
- This role is exempt from the Rehabilitation of Offenders Act
- DBS check required
