RN - Case Management

Job Details:

• Hospice case management position serving the Oshkosh/Green Bay market

• Work in community settings including patient homes, family member homes, assisted living facilities, group homes, nursing homes, and hospitals

• Remote/Field work with caseload of 13-15 patients

• Complete 5-6 visits per 8-hour shift

• Inpatient hospice work with 1:6-9 patient ratio depending on shift

• Care for hospice patients with life expectancy of 6 months or less

• Patient diagnoses include 40% cancer cases and 60% other medical issues

• 2 years of RN experience required

• First-time travelers accepted

• Required certifications: BLS and state license

• Must have IV skills, chest tube maintenance/pleurex drain experience

• Perform catheter insertion, tube feedings, and wound care including wound vac

• Work with PIV, midline, and central lines

• Provide end-of-life care including pain and symptom management

• Experience with opioids and opioid equinalgesic table required

• Document on EMR platform

• Must be independent critical thinker while knowing when to seek assistance

• Flexibility required for various care settings
