Description

Primary Registered Nurse — The nurse is the primary case manager for patients who live in the community and within facilities. The primary nurse collaborates with float nurses to provide safe, consistent, and compassionate care to both the patient and their family. Flexibility, teamwork, and exceptional communication are essential. The primary nurse collaborates with the interdisciplinary team to provide updates and achieve high-quality patient standards.

When caseloads are lighter, you will support the team by assisting with patient visits and transitions, serving as a Float Registered Nurse as needed. This role is in the Oshkosh territory and may travel outside of the territory.

Schedule

Full-time, 40 hours per week (7:00 am – 5:00 pm, 10-hour shifts). Requires working a minimum of every 3rd weekend; currently scheduled every 9th weekend. Includes 10–15 rotating on-call shifts per year and approximately 1–2 holidays annually.

Job Duties
  • Collaborate with primary nurse to provide clinical and supporting case management based on patient’s diagnosis and end-of-life symptoms.
  • Complete initial, comprehensive, and routine assessments of patient and family needs.
  • Respond to urgent patient/family needs in collaboration with triage nursing staff.
  • Initiate crisis care as needed.
  • Develop, implement and review care plans for patient and family. Update care plan as the needs of the patient and family change.
  • Report appropriate findings and significant changes to the primary team, attending physician and primary caregiver.
  • Communicate with all interdisciplinary team members.
  • Teach family members patient care skills and encourage their participation including, but not limited to, pain control, medication management, dressings, hygiene and nutrition.
  • Provide education to patient/family members on disease progression, symptom management, medication and other related patient care needs.
  • Administer and titrate medication according to physician/nurse practitioner orders.
  • Record all received orders to the Electronic Medical Record and initiate follow up as needed.
  • Delegate nursing duties to LPN and supervise care provided by CNA.
  • Prioritize daily work and adjust to the needs of patients, Unity and other staff.
  • Complete new patient admissions as needed.

Requirements / Qualifications
  • Current unrestricted Registered Nurse licensure from the State of Wisconsin
  • Minimum of 2 years RN experience required, equivalent nursing experience may be considered
  • Valid Wisconsin Driver’s License, reliable transportation and proof of current automobile insurance coverage
