What you’ll be doing :
Conducts comprehensive nutrition assessments for patients.
Develops and implements individualized nutrition care plans
Provides counselling and education and in-services to patients, using trauma-informed and patient centered approach
Documents as per HHC and college standards, including assessments, interventions and outcomes in patient medical record
Collaborates with the clinical team to provide the best nutritional care for patients
Advises and develops custom menus, snacks
Promotes healthy eating behaviors
What we’re looking for:
Bachelor’s degree in Human Nutrition and Dietetics from an accredited post-secondary institution
Valid registration with College of Dietitians of Ontario is required, member of Dietitians of Canada preferred
5 years’ previous experience in Older Adult, and Eating Disorders
Experience with mental health and addictions populations
Collaborative, service-oriented, innovative and accountable
Commitment to working within Homewood’s Health and Safety Standards
Practice in accordance within the ethical guidelines of the Dietetics profession
