$90,000 – $115,000 per year, plus a $5,000 signing bonus.

The Home Health Physical Therapist evaluates, plans, directs, and administers physical therapy treatments. They supervise assistants, delegate tasks, and use prescribed treatments to restore function, prevent disability, and help patients achieve maximum performance within their capabilities.

• Current Physical Therapy license in the state of practice
• At least 2 years of PT experience
• Ability and desire to work independently
• Experience with Kinnser/Wellsky preferred
• Effective verbal and written communication and time management skills
• Valid driver’s license with proof of current insurance and reliable transportation
• Valid AZ Fingerprint Clearance Card
• Current CPR certification and proof of negative TB test
• Ability to pass a criminal background check
