Job Introduction

Are you a compassionate and experienced Registered Nurse looking to use your clinical expertise in a dynamic, coordination‑focused role? HCRG Care Group is seeking a Band 5 Triage Nurse to join our dedicated team, helping us deliver safe, timely and effective community care.

About the Role

As a Band 5 Triage Nurse, you will play a central role in managing the flow of referrals into the District Nursing service. Acting as the ‘District Nurse of the Day’ , you will:

  • Triage incoming referrals , determining clinical priorities, levels of need and risk.
  • Coordinate onward referrals to the District Nursing team, ensuring patients receive the right care at the right time.
  • Monitor and track all referrals , reducing attrition and supporting seamless continuity of care.
  • Work closely with our administrative team to maintain accurate and timely referral information.
  • Provide safe, effective decision-making in an office-based, non–patient facing environment .
About You

We’re looking for someone who is:

  • An experienced Registered Nurse , qualified to Band 5 level.
  • Confident and experienced in clinical decision-making.
  • Highly organised, able to prioritise and manage multiple demands.
  • A strong communicator who enjoys working collaboratively across teams.
  • Motivated to support high-quality community healthcare through robust triage and coordination.
