As a Band 5 Community Nurse, you will be an essential part of our
24hour district nursing service, delivering high-quality, holistic care to patients in their own homes. Working under the direction of the District Nursing Team Manager/Leader, you will:

• Carry delegated responsibility for theassessment, planning, implementation and evaluation of evidence based nursing care.
• Provide skilled clinical care that supports patients to maintain independence and quality of life at home.
• Supervise and support team membersin the absence of the District Nursing Team Leader when required.
• Work collaboratively with multidisciplinary colleagues to ensure safe, coordinated and person-centred care.

We are looking for someone who is:

• An experienced Registered Nurse with a passion for community healthcare.
• Confident in delivering holistic, evidence based nursing care.
• Able to work autonomously while remaining a strong team player.
• Skilled in communication, assessment and clinical decision making.
• Flexible, compassionate and committed to delivering high standards of care.
