About SALT Outreach Inc

SALT Outreach Inc. operates a mobile drop‑in center for people experiencing homelessness. We operate five days a week in downtown Orlando, serving up to 200 people per day through 12 different services, which include case management and mental health counseling. Our mission is to share Christ’s love by serving the unsheltered through comprehensive mobile drop‑in centers. Our vision is that every mafaiunal area in the U.S. has a comprehensive drop‑in center for people experiencing homelessness.

Description

Field/Street Outreach component:

Travel would be required three days per week for up to four hours per day on Monday, Wednesday, and Friday. The Field/Street Outreach Case Manager will explore camps, wooded areas, parks, abandoned buildings or other places in the Sanford area where the unsheltered may frequent in hopes of connecting them to services. Effective street outreach reaches people who might not otherwise seek assistance or come to the attention of the homelessness service system.

Thursdays are flex days in which you would conduct follow‑up appointments, take clients to their own appointments, and/or search for resources within Seminole County within a desk space at our main location in Orlando, Florida.

Responsibilities And Duties
  • Field/Street Outreach – explore unsheltered locations such as parks, encampments, cars, abandoned buildings, wooded areas, and identify unsheltered clients.
  • Complete engagement forms with unsheltered clients to determine needs and inform them about SALT services and other services in the community that are available to them.
  • Possibly provide transportation/transportation resources for clients to be able to participate in programs and services.
  • Referrals – refer clients to organizations to obtain services and resources not offered through SALT.
  • Hold clients accountable to referral and follow up with client, system, or organization to confirm referral was followed through.
  • Keep and monitor a log of each organization in Central Florida that supports the unsheltered. Update the log as needed.
  • Case Management – identify candidates eligible for case management services, assist each client with creating a plan, prioritizing their needs by turning them into steps to help each client accomplish their goals, and empower them to pursue their goals as it pertains to obtaining housing or employment.
  • Documentation – keep detailed internal notes for each client in a secure location; ensure privacy and confidentiality of all notes taken; obtain signed authorizations to obtain and disclose information before entering it in HMIS; documentation will be done through the Homeless Management Information System and SALT’s internal note system to support the Continuum of Care and must be completed within one day of sitting down with a client;

    monthly reports may need to be produced and presented to the management team.
  • Supervision – may supervise up to two social work interns, assign cases, assist in interviewing and orienting new interns, conduct training with interns individually or as a group, and serve as a resource and coach for interns throughout their internship experience.
  • Transportation – drive to designated areas for street outreach at the direction of senior case managers, use company vehicles for transportation to services when applicable, and may use company vehicles to purchase supplies or services for clients.
