• Liaises between Care Management and Hospitalist providers to address patient challenges.
• Communicates daily with Hospitalist providers to review and update patient discharge plans.
• Participates in interdisciplinary meetings to discuss patient care and discharge strategies.
• Communicates information with providers and facilitates changes in plan of care as directed by physician.
• Acts as an escalation point for complex cases and issues.

• Bachelor’s of Nursing [Required]

• Nursing

• 3+ experience as a RN [Required]
• Acute care experience [Preferred]
• Experience in Case Management, Managed Care, Discharge Planning and Utilization Management [Preferred]

• Registered Nurse (RN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
