Licensed Practical Nurse - Patient Aligned Care Team; Orlando

Overview

The PACT LPN Clinical Associate carries out the most complex practical nursing care assignments under the general review of a Registered Nurse (RN) in the outpatient clinic. The LPN uses a systematic problem‑solving process to provide individualized, goal‑directed nursing care for multiple patients with predictable health care needs.

• The LPN may contribute to the plan of care by collaborating with interdisciplinary team members, the patient, and the patient’s family.
• Collect data and information, recognize changes in conditions, and report to the RN supervisor or other clinical supervisor to assist in identifying problems and formulating goals, outcomes, and patient‑centered plans of care developed with patients and families.
• Participate in the nursing process by appraising the patient’s status and by implementing established teaching plans for patients and families with common health problems and learning needs.
• Suggest modifications to the plan of care as necessary.
• Facilitate continuity of care by making appropriate referrals and demonstrate competence in delivering holistic care through collaborative strategies with others.
• Operate at GS‑7 level with broader knowledge and skills for increased involvement in development and modification of the Veterans plan of care, exercising independent technical judgment under general supervision.
• Work schedule:
  
  7:30am – 4:00pm, Monday–Friday (flexible to meet organizational needs).

• Licensed Practical Nurse (LPN) with the ability to perform nonstandard and specialized nursing activities under supervision.
• Evidence of broader knowledge and skills for increased involvement in care plan development and modification.
• U.S. Citizenship required. Proficiency in written and spoken English.

• Selective Service registration for males born after 12/31/1959.
• Participation in seasonal influenza vaccination program is required for Department of Veterans Affairs Health Care Personnel (HCP), with exemptions possible for medical or religious reasons.
