SENIOR COMMUNITY HEALTH NURSE - - State of Florida, Orlando, FL

The CMAT Registered Nurse functions as an integral member of the Children’s Multidisciplinary Assessment Team (CMAT) to provide pediatric nursing expertise, assessment and evaluation for medically necessary long-term care services for Medical Foster Care (MFC), Skilled Nursing Facilities (SNF) and Model Waiver. This position is designated as on-site, with possible remote start before on-site transition.
- Assessment
:
Review CMAT referrals for completeness and appropriateness; obtain missing documentation; request/obtain medical records; conduct nursing assessment interviews; document findings; complete comprehensive, family-centered clinical assessments within required time frames. - Communication
:
Provide clear verbal and written communication with CMAT and stakeholders; coordinate CMAT staffing schedules; engage and support parental participation and family-centered approaches; provide nursing education and technical assistance; participate in community outreach. - Implementation
:
Contribute nursing perspective to multidisciplinary team for eligibility determinations; review and explain medical terminology; support team consensus on LOC recommendations for MFC and SNF; document CMAT activities and ensure timely information sharing. - Outcome Evaluation
:
Document CMAT staffing outcomes, update medical information, disseminate staffing summaries to stakeholders, and ensure data integrity across electronic systems. - Quality Improvement
:
Review data and documentation for accuracy; participate in QA/QI activities and PDSA cycles; support training and follow-up to improve CMAT functions. - Other duties
:
Include coverage of MFC RN duties as assigned.
- Minimum Qualifications
:
Licensure as a Registered Professional Nurse (Florida Statute 464) or eligible to practice (F.A.C. 210-8.27 or 8.27). Proficient with technology and electronic health records. - Preferred Qualifications
: 1-3 years pediatric nursing experience. - Required Knowledge, Skills, and Abilities
:
Effective verbal and written communication; planning and organizational skills; data collection and analysis; computer literacy; knowledge of nursing care for children with special health care needs; knowledge of foster care system and child/family resources; nursing assessments; clinical nursing activities; ability to provide training and education; strong nursing documentation skills.
Location:
3319 Maguire Blvd Suite 125, Orlando, FL 32803. Travel: up to 10%.
