Responsibilities

The Licensed Practical Nurse in Personal Care Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care by communicating with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

Deliver high-quality clinical services within the scope of practice and infection control standards, following the plan of care and coordinating with the care team from admission to discharge.

Complete clinical nursing assessments as required by federal and state program requirements and payers.

Verify patient eligibility and medical necessity for services per payer and agency policies.

Develop and update individualized care plans in accordance with federal and state requirements, collaborating with community providers.

Ensure the plan of care's frequency and duration meet patient needs, revising as necessary with physician approval.

Experience and License Requirements

A minimum of one year of RN experience; PEDS experience preferred.

Current RN licensure in the practice state.

Valid driver’s license, vehicle insurance, reliable transportation or access to public transit.

Current CPR certification.
