Registered Nurse Days - In Patient Unit - Sign On Bonus
Responsibilities include assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to patients and families.
Qualifications- Current license as RN in the state providing care, current active single-state out-of-state nurse license, or current active multistate/compact nurse license
- Minimum one (1) year in-patient nursing or hospice experience preferred for external candidates; for internal applicants, minimum of 6-months of LPN experience with hospice/home health within an affiliate preferred. New grads may be considered in IPU only.
- Previous experience with an EMR/EHR system preferred
- Ability to use equipment with visual and auditory mechanisms
- English communication skills (verbal and written)
- Flexible schedule to meet the needs of Hospice House patients
- Ability to manage the emotional stress of caring for terminally ill patients and families
- Ability to physically access patient rooms and other areas within the service area
- Ability to perform the essential functions and physical requirements of the job with or without reasonable accommodation
- Active BLS for healthcare professionals
- Valid driver7s license and automobile insurance per Company policy with ability to travel off-site as needed
- Represent the Company professionally through care delivered and services provided to all clients.
- Comply with all state, federal and local regulations, policies, procedures and standard practices.
- Maintain confidentiality of patients, families, colleagues and sensitive information.
- Use resources responsibly and participate in quality improvement activities.
- Promote the Company through community and professional involvement.
- Support company research and educational activities.
- Share expertise with co-workers and participate in performance improvement activities as appropriate for the position.
- Provide and manage direct care to patients and families as part of IDG, incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing interventions and follow-up.
- Coordinate the Plan of Care reflecting the patient’s evolving needs.
- Educate patient and family about disease process, palliative interventions, care giving, and safety practices.
- Report changes in the patient’s condition to appropriate IDG members or other health professionals.
- Evaluate hospice referrals/admissions for level of care appropriateness with the IDG.
- Attend patient deaths; prepare body for transport and oversee disposal of medications; provide appropriate bereavement interventions with psychosocial staff.
- Communicate concisely with physicians, staff, patients and families; encourage input from all disciplines.
- Supervise nursing team members as applicable (LPNs, Hospice Aides, and contract nurses).
- Performs other duties as assigned.
- Some locations may require cross-training to field positions when census requires flexible staffing.
