Position Summary: Orlando Health’s South Lake Hospital is a comprehensive medical and surgical acute care facility serving Lake County. We seek an RN Care Manager to promote and facilitate effective management of hospital resources from admission to discharge.

Key Responsibilities
  • Assess patients’ needs and develop or revise individualized care plans based on clinical progress.
  • Promote cultural competency and build respectful, collaborative relationships with all team members, patients, and families.
  • Act as a patient and family advocate, ensuring their needs and concerns are addressed.
  • Apply knowledge and clinical skills necessary to provide appropriate care in the intermediate care unit.
  • Serve in leadership roles (e.g., preceptor, charge nurse, unit educator) to support staff development.
  • Collaborate with medical staff and the interdisciplinary team to develop and implement comprehensive care plans.
  • Model clinical excellence and professionalism, supporting hospital and nursing department goals.
  • Understand growth and development principles across the lifespan and assess patient data accordingly.
  • Coordinate and delegate care among team members for a defined patient group.
  • Document patient care consistently and accurately, complying with regulatory standards.
  • Maintain competence in nursing skills and use of patient care equipment specific to the intermediate care unit.
  • Prioritize patient care based on critical needs, using evidence‑based practices.
  • Utilize problem‑solving skills to identify, address, and resolve issues promptly.
Other Related Functions
  • Contribute to the continuous improvement of health‑care practice and organizational outcomes.
  • Participate in unit or organization‑level committees of nursing practice and performance improvement.
  • Engage in peer review, mentoring, and coaching regarding professional practice.
  • Use supplies efficiently and maintain a clean, safe, organized work area.
  • Attend staff development, in‑services, department meetings, and nursing committee meetings.
  • Partner with nursing leadership to identify professional development needs.
  • Assume responsibility for own professional development and continuing education.
  • Perform other duties as assigned.
Qualifications
  • Education: Graduate of an approved school of nursing.
  • Licensure/Certification: Current Florida RN license; BLS/Healthcare Provider certification (required; within 90 days of hire).
  • Experience: Three (3) years in chronic disease management, care management, care coordination, utilization management, or acute clinical care.
