Working Title:

SENIOR VOCATIONAL REHABILITATION COUNSELOR -

This position requires daily in-office and field work.

Responsibilities:

  • Professional counseling position providing case management services to both pediatric and adult individuals with brain and/or spinal cord injuries.
  • Applicant Eligibility Determination: determines program eligibility through assessment of medical and residency status, potential to reintegrate into the community, and other program requirements as defined in the Brain and Spinal Cord Injury Program’s Policies and Procedures.
  • Provide case management services independently for a caseload ranging from approximately 10-15 referrals per month.
  • Contact referral sources to ensure all documentation is received to determine eligibility; this may take weeks or up to the 60-90 day policy before placing the applicant in service or closing.
  • Contact client or family member to introduce the program, explain eligibility, and arrange a time for the Initial Interview.
  • Case Management:
    Conduct initial interviews to determine service needs; develop with client/family a plan of services including assessment of available third-party benefits; implement, monitor, and evaluate progress toward community reintegration; identify service providers; work with VR Tech to authorize services; provide referrals to other agencies; respond to client, family and provider needs; ensure activities follow program policies and procedures.
  • Assess short-term and long-term needs and establish case management objectives for a caseload of 30-50.
  • Perform Initial Interviews via phone or field-based scenarios; coordinate discharge needs with families and referral sources; arrange delivery of approved DME with vendors and obtain bids if applicable.
  • Determine funding sources to create individualized Care Plans; set up outpatient therapies with designated rehabilitation centers.
  • Facilitate pediatric cases by consulting with therapists, schools, and doctors to ensure collaboration and provide ongoing education through mandated programs (e.g., ESE IEP accommodations and services, 504 Plans, Homebound schooling).
  • Act as liaison and advocate between clients/families, physicians and facilities/agencies, and guardians as applicable.
  • Manage active cases based on intensity and acuity; coordinate with specialists as needed for pediatric cases to ensure smooth integration.
  • Maintain ongoing communication with clients, family, physicians and other providers; use monthly progress notes and visit notes to adjust Care Plans as needed.
  • Assess care options including use of benefits and community resources to update Care Plans.
  • Answer residents’ questions about care, treatment plans, and illness progression to ensure they feel safe and secure in care.
  • Transfer appropriate cases within and between regions with status and needs communicated via transfer summaries.
  • Conduct proper closures following program policies and procedures.
  • Maintain accurate and real-time records of all case management activities in the RIMS System using HIPAA guidelines.

Documentation: Documents all activities related to the applicant/client’s case including case notes, medical records, contacts, and consultative reports in the Rehabilitation Information Management System (RIMS). All documentation activities follow program policies and procedures. Budget:
Monitors and manages budget for the caseload as outlined in program policies and procedures.

  • Respond to legal and Social Security requests in a timely manner; this may save money for the program through reimbursements and expedites the client’s financial status.

Liaison

Activities:

Develops relationships with hospital personnel, community partners, service providers, and other professionals. Provides technical assistance related to client needs and participates in or leads in-service briefings or program overviews as requested. All activities must be approved in advance by the Operations Services Manager.
