The Licensed Clinical Social Worker assists patients and their families in understanding a particular illness, work through the emotions of a diagnosis, and providing counseling about the decisions that need to be made. Works in collaboration with CHC clinical staff (doctors, nurses, and allied health professionals) using case management skills to help patients and their families address and resolve the social, financial, and psychological problems related to their health condition.
QualificationsEducation
- Bachelor s Degree in healthcare or other related field.
- Master s Degree in Social Work from a Council on Social Work Education-accredited school of social work.
- LCSW - License as Clinical Social Worker.
- Current BLS certification required or must be obtained within the first 30 days of employment.
- Must obtain the Functional Assessment Rating Scale (FARS) and Children s Functional Assessment Rating Scale (CFARS) certification within the first 2 weeks of employment.
- 2 years of previous experience in a healthcare setting is required.
- Experience in an ambulatory health center preferred.
- Requires someone familiar with and sensitive to Hispanic culture, family customs, and child-rearing practices.
- Familiarity with multicultural groups, previous work experience in a multicultural setting, and willingness to enroll in language and multicultural coursework to supplement training if necessary.
- Previous supervisory experience preferred.
- Knowledge of EHR software or equivalent, preferred.
- Bilingual in English and Spanish preferred.
- Maintain confidentiality regarding privileged administrative and client information in a professional manner.
- Intermediate knowledge of Microsoft Office, including Word, Excel, and Outlook.
- Able to work flexible hours, including weekends as needed.
- May be required to travel to other CHC centers.
