Primary Care Physician Role

  • Accountability for outcomes – strong clinical care and cost‑effectiveness for up to 450 patients.
  • Health coaching – set short and long‑term goals, partner with patients, and follow up.

Metrics regularly measured include patient admissions per thousand, appointment slots usage, CGCAHPS, clinical gaps closures, and medical cost measures. Each PCP has goals for these metrics, collaborating with center and market leadership and care teams.

Job Duties and Responsibilities
  • The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults.
  • The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, while staying involved in the detailed care.
  • It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for Chen Med or not).
  • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
  • The PCP leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
  • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound—the PCP will engage with the transitional care team and others including case managers, acute and transitional‑care physicians, and other resources that may be available depending on the market.
  • PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
  • Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.
  • Performs other duties as assigned and modified at the manager’s discretion.
Knowledge, Skills & Abilities
  • Availability and accessibility for patients to build trust from their patients. It is expected that PCPs will be as available to patients as possible by being open and available for walk‑in visits and answering phone calls and messages in a timely manner.
  • Service orientation – PCP’s provide care that they would want for a family member or for themselves to each patient at every interaction.
  • Evidence‑based medicine – The PCP remains updated on evidence‑based medicine, but also recognizes that factors outside of traditional medicine, like lifestyle and nutrition, have a large impact on patient health outcomes. The PCP stays up to date on clinical, nutritional, and lifestyle‑based interventions to improve outcomes.
  • Physician leadership – continuous development and improvement of leadership skills for the benefit of patients, their team, their center and the company.
  • Quality – Highest quality of care with a willingness to work with the care teams towards achieving high quality outcomes and quality measures. At the same time, PCPs will always be looking for continuous and systematic improvement of their practice and the operations of their center.
