DefinitionUnder general direction of the Executive Director, the CHW Program Manager provides licensed oversight, leadership, and program development for Community Health Worker (CHW) services across multiple initiatives. This position is responsible for supervising CHWs, guiding care planning and service delivery, strengthening community partnerships, and leading the development and expansion of the CHW program in alignment with organizational mission, values, and community needs.
The CHW Program Manager serves as a people-centered leader who supports staff success through servitude leadership, collaboration, accountability, and continuous improvement.
Distinguishing CharacteristicsThis position emphasizes program development, leadership, and supervision rather than direct service delivery. The CHW Program Manager functions as a program architect and culture-setter, balancing licensed medical oversight with community-centered practice. The role requires strong relational leadership, adaptability across initiatives and counties, and the ability to support frontline staff without micromanagement.
SupervisionSupervision Received:
Receives direction from the Executive Director.
Supervision Exercised:
Exercises direct supervision over Community Health Workers and may supervise additional staff as programs expand. This position has hiring, performance management, and disciplinary authority consistent with organizational policy.
- Lead the development, implementation, and expansion of CHW programming across multiple initiatives, including but not limited to ECM, housing, employment, children and families, and grant-funded programs.
- Establish clear program structures, workflows, and expectations that support high-quality, community-centered services.
- Collaborate with executive leadership to align CHW programming with organizational strategy, funding requirements, and community priorities.
- Support the development of CHW-led health education groups and community-based interventions.
- Provide strengths-based supervision, coaching, and support to CHWs using a servitude leadership approach.
- Foster a team-oriented work environment that values collaboration, accountability, learning, and staff wellness.
- Conduct onboarding, training, and ongoing professional development for CHWs.
- Address performance concerns through supportive coaching and corrective action when necessary.
- Provide licensed medical oversight related to care planning, service coordination, and quality assurance.
- Offer clinical consultation and guidance to CHWs within scope of licensure.
- Support quality improvement efforts to ensure services meet program, funder, and regulatory requirements.
- Review documentation, care plans, and service delivery for accuracy, completeness, and quality.
- Build and maintain strong relationships with community partners, providers, managed care plans, and county agencies.
- Serve as a clear point of communication regarding program expectations, roles, and collaboration.
- Represent the organization in meetings, coalitions, and community forums as needed.
- Model organizational values, including humility, transparency, accountability, and commitment to community health.
- Participate in leadership team meetings and contribute to organizational planning and continuous improvement.
- Adapt to evolving program needs and support innovation across communities and counties.
