Position: Wound Care Coordinator - Full-Time 1st Shift

Full Time Role

Salary: 81,900k - 88,000k (Commensurate with experience)

5 days per week: 8am – 4pm schedule. Part of the On‑call manager rotation.

We are looking for a stellar nurse to continue wound care necessities for high level of care as we continue to enrich the lives of our residents.

What you bring to the table:
  • Must have RN license in good standing in Illinois, will consider LPN with senior living experience.
  • Minimum 3 years industry experience
  • Leadership and managerial experience.
Duties and Responsibilities:
  • Wound Care Coordinator Registered Nurse is responsible for the overall management of the skin/wound care program.
  • Wound Care Coordinator Nurse addresses skin tears, rashes, pressure areas, changes in skin condition, follows up with new orders, treatment plans & assessments. Change plan of care as needed, communicate changes/recommendations to attending physician, staff, and families.
  • RN completes weekly wound rounds with physician and documents resident treatment plans, completes the admission assessment, MDS data as it pertains to the program, updates care plans with each MDS assessment as needed.
  • Incident/Accident in regard to skin alterations with a follow up investigation and report as needed.
