RN Coordinator - Infusion

Overview

RN Coordinator - Infusion at Providence Health and Services is responsible for planning and coordinating new patient referrals, and providing phone triage and case management for infusion therapy patients, primarily onsite. This role collaborates with providers, patients, caregivers, physicians, and payors across Oregon and southwest Washington. Active RN license and experience in home infusion nursing are required, with opportunities for clinical care and coordination in home and clinic settings.

• Plan and coordinate new referrals for infusion therapy.
• Provide phone triage for existing patients.
• Deliver on-site nursing services and case management related to infusion therapy in the home, clinic, or alternate living site.
• Collaborate with providers, patients/caregivers, physicians, and payors to coordinate care.
• Work in the states of Oregon and/or southwest Washington.

• Required:
  
  Oregon Registered Nurse License or Washington Registered Nurse License (upon hire).
• Required:
  
  National Provider BLS – American Heart Association within 30 days of hire.
• Required:
  
  Licensed in states where care is delivered.
• Required:
  
  3 years of home infusion nursing experience.

• Bachelor's Degree in Nursing.
