Interim Assistant Nurse Manager, Care Coordination / Discharge Planning
for a W2 Contract position based in Clackamas, OR.
Full time, Monday through Friday, 8am - 4pm
Assignment Duration:13 Weeks (possibility of extending)
Compensation:This position offers a daily rate of $710 to $730 which equates to $3,550 to $3,650 per week (based on a 5-day / 40-hour workweek)
Per Diems:
Lodging: $136 per day and Meals & Incidentals: $80 per day
Per diems are paid daily, 7 days a week, totaling $1,512 per week
Auto Stipend: $25 per day
Stipend is paid daily, 7 days a week, totaling $175 per week
Total Weekly Compensation (Daily Rate + Per Diems + Stipend): Between $5,237 to $5,337 per week

Summary:Supports the Nurse Manager in the delivery of coordinated care across the continuum, including utilization review, discharge planning, and care transition activities. Provides clinical and operational leadership to RN Case Managers, Social Workers, and Care Coordination staff to promote safe transitions, optimize throughput, ensure regulatory compliance, and support organizational quality and financial goals.
Essential Functions:Oversees daily care coordination and discharge planning activities to ensure timely, safe patient transitions across the continuum.
Facilitates interdisciplinary discharge planning rounds and ensures barriers to discharge are identified and escalated appropriately.
Collaborates with physicians, nursing, social services, and external partners to align discharge plans with clinical needs and patient/family goals.
Ensures documentation of discharge plans, continuing care needs, and handoffs meet regulatory, payer, and organizational requirements.
Supports utilization review processes to ensure compliance with medical necessity criteria and appropriate level‑of‑care determinations.
Coordinates with Physician Advisors and payers on authorization, concurrent review, and denial prevention activities.
Monitors patient throughput, length of stay, avoidable days, and discharge timeliness; escalates bottlenecks as appropriate.
Ensures adherence to CMS, Joint Commission, and health plan requirements related to UM/CM.
Provides direct supervision, coaching, and support to RN Case Managers, Social Workers, and Care Coordination team members.
Participates in hiring, onboarding, scheduling, competency validation, and performance evaluation.
Leads team huddles, rounds, and care coordination workflows; promotes staff engagement and interdisciplinary collaboration.
Assists the Nurse Manager with unit operations, staffing decisions, resource allocation, and departmental projects.
Supports quality improvement initiatives related to LOS, readmissions, denials, patient flow, and care transitions.
Ensures compliance with federal, state, and accreditation standards, including CMS Conditions of Participation.
Participates in audits, documentation reviews, and corrective action plans as required.
Promotes patient‑centered care practices and culturally sensitive communication.
Qualifications:BSN or BA in health care related field required. Masters preferred. Experience in lieu of degree will be considered.
Graduate of an accredited school of nursing required.
BLS and ACLS certifications required.
Clinical nursing experience relevant to department (around 3‑4 yrs), including 2 years leadership experience required.
