LPN On Call

Overview

Participates with the Health Care Team in delivering quality and customer focused care to patients in a manner that reflects Kaiser Permanente's mission, vision and values. Provides nursing and administrative support to clinicians and other health care team members to offer a variety of health care services to patients. Performs clinical tasks and procedures under the direction of a clinician, within the scope of the LPN's professional training and individual competency.

Works under the clinical supervision of the clinician or registered nurse and under the general direction of the designated supervisor.

• Clinical Tasks: Prepare and organize patient visits for known reasons/procedures. Ensure all appropriate tests, clinical information, and order entries are ready for clinician review. Perform focused/limited nursing assessment and triage on routine or self-limiting conditions as directed. Recognize when to defer to/assist with comprehensive assessment for complex conditions. Use standardized tools to identify and address health maintenance gaps and alert the clinician as needed.
  
  Document nursing care in the electronic medical record using standardized formats. Ensure exam rooms are stocked and clean, and provide health teaching for routine conditions as appropriate.
• Clinical Procedures: Perform diagnostic tests, therapies and procedures within scope of practice, following department standards and safety protocols. Examples include vital signs, screening and diagnostic tests, medication administration, wound care, therapies, orthopedic devices, catheter care, minor procedures, laboratory specimen collection, and instrument setup.
• Practice Management: Maintain and adjust clinicians’ schedules for efficient patient flow. Respond to incoming messages, coordinate follow-up duties, processing prescriptions and forms, and input patient data into the EMR. Support patients with complex health needs in navigating the health care system.
• Teamwork and Professional Development: Start workflow per team agreements, adapt to urgent demands, assist with medical emergencies, mentor peers, report errors or hazards, and complete annual competencies.

Basic Qualifications: N/A

• Graduate of an accredited school of nursing.

• Credentials from multiple states required; primary state credentials at hire, secondary state credentials post-hire.
• Practical Nurse License (Washington) within 6 months of hire OR Compact License:
  Practical Nurse within 6 months of hire.
• Practical Nurse License (Oregon) within 6 months of hire.
• Basic Life Support.

• Demonstrated clinical nursing skills.
• Interpersonal and customer service skills; assessment required for external candidates.
• Strong written and verbal communication skills.
• Computer literacy and keyboarding skills.
• Problem-solving, priority setting and good judgment.
• Ability to work independently and under pressure.

• Two or more years of nursing experience in assigned department within ambulatory care.
• IV therapy experience or recent certification.
• Familiarity with electronic medical records.
• Current knowledge and skills in providing routine care and education in the assigned department, or successful completion of relevant competencies.
