Job Summary The RN Team Leader assumes a lead role in collaboration with a clinician partner to assist in the timely, appropriate and safe transition of patients from hospital, emergency departments, home health/hospice, and outpatient clinics to other care settings across the continuum. The RN Team Leader supports and assists with the development of a high functioning self-directed team. Practices and demonstrates skills that foster professional commitment of the team members to function to their level of licensure and training, make care decisions utilizing appropriate tools, and assumes accountability for providing safe and timely transitions of care which support Kaiser Permanente’s mission, vision and values.
EssentialResponsibilities
- Practices clinical and leadership skills. Evaluates and prioritizes staffing needs while collaborating with manager/department assistants.
- Collaborates, advocates, identifies scope, workflow, policy/procedure involvement. Handles member concerns. Mentors and coaches team members with issues and concerns related to patient care and workflow.
- Demonstrates critical thinking and delegation of appropriate work. Communicates effectively with patients, clinicians, interdepartmental and intradepartmental staff.
- Participates in the hiring, training and evaluating process. Facilitates orientation and training needs. Assists in the assessment and monitoring of clinical competencies of the team. Provides input into performance evaluations.
- Acts as mentor and coach in assisting team members with training or educational needs. Demonstrates personal commitment to continuous learning and professional development.
- Coordinates communication flow within the team and with other departments within KP and plan hospitals. Participates in communicating department issues locally and regionally. Facilitates information flow during emergencies.
- Communicates work team issues to the manager on a regular basis. Communicates team performance in meeting clinical and service targets.
- Models and monitors adherence to Quality of Service Standards and Behavioral Guidelines, ethical and legal behavior in all professional activities. Supports KP policies, legal considerations and professional ethics.
- Adheres to and assists in monitoring the team’s adherence to HIPAA, confidentiality policies and Principles of Responsibility (POR). Develops and maintains escalation paths for interdepartmental or patient/family issues.
- Demonstrates knowledge of and facilitates national and regional strategic initiatives at the local level. Maintains relationships with facilities and external partners. Ensures staff has access to facility or systems to support job duties.
- Provides clinical expertise and professional nursing leadership to support the health care team in meeting the needs of the patient and safe and timely transition in care across the continuum.
- Understands and participates in population-based care activities supported by an interdisciplinary health care team. Maintains a safe and therapeutic environment for patients, clinicians and staff. Demonstrates proficiency in using computer-based information and clinical tools to determine appropriate level of care.
- This work is in addition to CARE COORD, OR COORD, SNF PLACEMENT - DUAL or other positions this job serves as a lead over.
Basic Qualifications
- Minimum two (2) years of acute hospital experience with progressive levels of responsibility
- Minimum one (1) year of care coordination or care management experience
- BSN or bachelor’s degree in a health-care related field or currently enrolled in a BSN program, or four (4) years of directly related experience.
- High School Diploma or General Education Development (GED) required.
- This job requires credentials from multiple states. Credentials from the primary work state are required itional credentials from the secondary work state(s) are required post hire.
- Registered Nurse License (Washington) within 6 months of hire OR Compact License:
Registered Nurse within 6 months of hire - Registered Nurse License (Oregon) within 6 months of hire
- Basic Life Support within 3 months of hire
- Demonstrated ability to interrelate with physicians, nurses, support staff, and patients in an interdisciplinary approach.
- Demonstrated ability to work as part of a team and as a constant patient advocate.
- Basic physical, psychosocial, functional assessment skills.
- Familiar with care processes and systems in settings internal and external to Kaiser Permanente.
- Familiar with and able to collaborate with Kaiser Permanente and community resources.
- Thorough knowledge of teaching and delegation, physical assessment and care planning, and appropriate utilization of acute hospital, long-term care, and home care resources.
