Position: Hospice Community Liaison / Admissions Specialist (Experienced Hospice & SNF Professionals)

Agape Hospice NW is hiring an experienced Hospice Community Liaison with established relationships with SNFs, hospitals, and discharge planners to support admissions and referral growth.

This is not an entry-level hospice position.

Agape Hospice NW is seeking an experienced Hospice Community Liaison / Admissions Specialist to support referral relationships, admissions, and community outreach.

This role is ideal for:

  • Hospice Community Liaisons, or
  • RN, LPN, or LVN clinicians with hospice, SNF, admissions, discharge planning, or care coordination experience

This position is designed for professionals who want to step away from bedside care and into a relationship-based, business-facing hospice role—while still making a meaningful impact for patients and families. If you are new to hospice or seeking a bedside nursing role, this position will not be a fit.

What You’ll Do

  • Build and maintain referral relationships with hospitals, SNFs, physicians, board & care homes, and discharge planners
  • Educate patients and families on hospice eligibility and benefits
  • Guide referrals through the hospice admissions process
  • Support ethical census growth through relationship-based outreach
  • Participate in community and referral partner events
  • Maintain accurate referral documentation and regulatory compliance

Required Experience (Non-Negotiable)

  • Minimum 3 years of hospice, SNF, or post-acute care experience
  • Strong understanding of hospice philosophy and admissions criteria
  • Experience working with families, providers, or discharge planners
  • Valid driver’s license and ability to travel within the service area

Ideal Backgrounds

  • You’ve worked a hospice territory and know how relationships drive census
  • You are comfortable working with discharge planners, DONs, and case managers without an intro period
  • RN, LPN, or LVN with hospice, SNF, case management, or discharge planning experience
  • Clinicians transitioning out of bedside care
  • Organized, self-directed professionals comfortable working independently in the field
