Overview
We are seeking an experienced Home Health RN to provide skilled nursing care to adult and geriatric patients in a home health setting. The ideal candidate will have 1+ year of skilled Home Health experience
, with OASIS proficiency required
, and the ability to work independently while delivering high-quality, patient-centered care.
- Specialty: Home Health
- Experience: Minimum 1 year of skilled Home Health experience (OASIS required)
- Certification: Active BLS required
- Charting System: Epic experience preferred
- Scrubs: Any color permitted
- Perform comprehensive assessments, including Start of Care, routine visits, recertifications, and discharges
- Provide medication administration and management, wound care, and patient education
- Accurately document care using Epic and comply with all clinical and regulatory standards
- Collaborate with interdisciplinary teams to support optimal patient outcomes
