Full Time, Exempt (Limited Duration). Salary: $34.73/hr.
- Responsible for completing individual nutrition assessments and nutrition therapy for patients identified as diabetic, pre‑diabetic, at‑risk, or other nutritional‑related disease, or as requested by SCHC providers.
- Provide support to the Diabetes and Community Health departments by assisting with outreach content around nutrition, scheduling, counseling, tracking, and reporting outcomes.
- Develop and implement individualized nutrition plans that consider medical and nutritional needs, laboratory results, cultural and personal preferences, finances, and the patient’s personal goals.
- Work with clinic medical providers and team members to coordinate care between the patient and provider.
- Serve as a nutritional resource for all staff members by compiling information, including pamphlets, booklets, articles, and other resources for use in nutrition programs.
- Make referrals to Diabetes Personal Trainers, Life Coaches, Tobacco Cessation, Community Health Advocates, and other local resources as appropriate; help with billing and streamlined case management by acting as the rendering provider.
- Teach group education sessions related to nutrition and/or oversee such training presented by other staff.
- Develop nutrition lesson plans, education handouts, pamphlets, and visual aids as deemed appropriate.
- Perform other duties as assigned.
Certified Nutritionist with a license to work in Oregon and Washington, or eligible to apply for an Oregon/Washington nutrition license. Minimum 2 years experience working in a clinical setting, preferably with people with diabetes.
Certified Diabetes Care and Education Specialist (CDCES) preferred, or an interest in obtaining certification.
- Proficient computer skills including Microsoft Office (Excel, Word, PowerPoint). Experience with Next Gen EMR preferred.
- Experience working with the Native American / Alaska Native community is preferred.
- Good oral and written communication skills with strengths in teamwork, cooperative problem solving, and customer service responsibilities. Ability to consistently convey a pleasant and helpful attitude by utilizing excellent interpersonal and communication skills to manage sometimes stressful and emotional situations.
- Knowledge of the concepts and requirements of managing and maintaining confidential information and communications.
- Knowledge of social, cultural, and economic factors of individuals and families as they apply to public health nutrition and the ability to adapt their food preferences into individual nutritional plans.
- A thorough knowledge of human nutrition and its relationship to health and disease.
- Must have experience providing nutrition education and counseling to patients, families, caregivers, and groups from diverse socioeconomic and cultural backgrounds.
- Certified Lifestyle Coach for the National Diabetes Prevention Program, or ability to obtain within 6 months of hire.
- Must be able to engage and persuade clients using a variety of tools such as motivational interviewing, Critical Conversations Framework, etc.
- Independently plan, manage, and organize work, and set priorities and goals for individual patients and the overall program.
- Must manage multiple projects and adapt to changing assignments and priorities.

- Effectively engage in clinic and community health functions to promote wellness through healthy behaviors and community improvements.
- Ability to make decisions independently in accordance with established policy and procedures.
- Proficiently use the Nutrition Care Process as a systematic approach to providing high-quality nutrition care for assessment and reassessment, diagnosis, intervention, and monitoring and evaluation.
