OverviewAre you passionate about supporting individuals experiencing housing instability? Clackamas County Health, Housing & Human Services (H3S) seeks two Outreach and Engagement Assistants to help people access Medicaid-funded eviction and homelessness prevention programs through Measure 1115. At H3S, you will help individuals facing housing instability navigate services in a diverse, collaborative environment. This role offers hands-on experience with trauma-informed care, eligibility assessment, and direct support to the community.
Responsibilities- Conduct phone and in-person assessments to evaluate eligibility for Medicaid-funded eviction and homelessness prevention programs.
- Engage with individuals in crisis to ensure they feel heard and supported during the assessment.
- Collect and accurately document client information in a secure database.
- Collaborate with program coordinators and caseworkers for eligibility verification.
- Provide clients with clear guidance on available support resources and services.
- Maintain compliance with program guidelines, regulations, and reporting requirements; ensure accurate and timely submission of documentation and reports.
- Participate in ongoing training to enhance knowledge of trauma-informed care practices, DEI principles, and program eligibility criteria.
- A minimum of two (2) years of related experience working with vulnerable populations, particularly individuals experiencing homelessness or housing instability.
- Demonstrated commitment to diversity, equity, and inclusion, with an understanding of systemic inequities and their impact on marginalized communities.
- Proficiency in data collection and documentation, with experience using database systems or case management software.
- Proficiency in Microsoft Office Suite (including Outlook, Teams, etc.).
- Knowledge of trauma-informed care principles and ability to apply them effectively in client interactions.
- Ability to work independently and collaboratively in a fast-paced, high-pressure environment, prioritizing tasks and managing time effectively.
- Strong communication and interpersonal skills to establish rapport with clients from diverse backgrounds.
- Sensitivity to confidentiality and privacy concerns, with a commitment to ethical standards and maintaining client confidentiality.
- Experience in Medicaid eligibility, social services assessment, or service screening roles.
- Familiarity with eviction prevention and/or homelessness prevention programs.
- Proficiency in a second language, particularly Spanish.
- Training or certification in trauma-informed care practices.
- Knowledge of state and federal regulations related to Medicaid and housing assistance programs.
- Must pass post-offer, pre-employment drug test.
- Must pass a criminal history check, possibly including a fingerprint check.
- Must pass a post-offer physical assessment; accommodations reviewed per state and federal law.
- Driving is required for county business; valid driver's license and acceptable driving record required.
- Duties may include assessments, engaging with clients, and explaining eligibility; conduct eligibility assessments for Medicaid eviction and homelessness prevention programs.
- Engage clients with a trauma-informed approach, showing empathy and respect.
- Apply a Diversity, Equity, and Inclusion (DEI) lens in interactions and decision-making to ensure equitable access to services.
- Document client information in the designated database in accordance with confidentiality protocols.
- Collaborate with program staff to review findings and determine eligibility for enrollment.
- Provide information and resources on services and community resources.
- Participate in ongoing training to enhance trauma-informed care, DEI, and eligibility criteria knowledge.
- Maintain compliance with program guidelines, regulations, and reporting requirements.
This position is included in the county's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours will be discussed with the selected candidate. The position is eligible for hybrid on-site/telework subject to department policy and business needs. Telework locations must be in Oregon or Washington;
Washington residents must perform 50% on-site.
Annual Pay Range: $55,865.83 - $70,451.55
Hourly Pay Range: $26.858571 - $33.870939
