The Homeless Solutions Coalition of Clackamas County (HSCCC) is building a large facility in Oregon City, called Caring Place, that will house the Father’s Heart Homeless Day Center and Emergency Shelter, a community health center and a variety of homeless services providers. The providers will integrate homeless services, health care, substance abuse treatment and mental health treatment through a collaborative model of alternative staffing, patient-centered access, and care coordination.
Caring Place is scheduled to complete construction and begin operation on site at the end of 2026. In order to plan staffing and services for implementation and to establish a baseline for future evaluation, HSCCC will conduct a study of demographics, health status, housing status, health care utilization, and patient attribution of the target population, the 3000 annual clients currently served by the Father’s Heart.
Pacific Source has provided funding for the research, and Providence CORE has offered to provide the supervision of a graduate student research intern. HSCCC is reaching out to Portland State University to find a qualified and interested graduate student to conduct this research, beginning in September of 2025.
The GRA will support a range of project tasks including:
- Completing data sharing agreements with healthcare, housing services and government partners, and maintaining good relations with partner data staff.
- Working with service providers and health plan payors and government payors to identify the baseline data points needed.
- Mining existing data sets to complete the baseline for the target population, including the databases of Clackamas County’s HMIS, Health Share, Providence, Kaiser, and Care Oregon.
- Identifying gaps in data and devising strategies, such as surveys, to complete those gaps
- Conducting and overseeing data collection if needed at the homeless day shelter
- Analyzing qualitative and quantitative data
- Completing a final baseline and report
- Participating in regular meetings as needed or assigned
