Summary

This position works as part of a multidisciplinary team to integrate a pharmaceutical care program into NHC primary care clinics to support the delivery of appropriate, safe, efficient and cost‑effective care. This position will provide services to patients and be the medication subject matter expert as part of the interdisciplinary care team.

Job Duties
  • Expands pharmaceutical care services to patients across the operating system at NHC.
  • Provides patient centered service and support a positive work environment.
  • Creates an ongoing relationship with each patient (and as appropriate with their families or caregivers) by providing access, continuity, education and comprehensive care via evaluation and treatment of NHC patients within the scope of his/her license.
  • Provides comprehensive medication review for patients with assessment of each medication for appropriateness, efficacy, safety and adherence.
  • Provides post diagnostic disease state management pursuant to Collaborative Drug Therapy Management Protocols.
  • Initiates, adjusts, and monitors changes in drug therapy based on established collaborative drug therapy management protocols or state granted prescriptive authority.
  • Provides health, wellness and medication education and recommends therapeutic lifestyle interventions.

    Orders, interprets, and monitors laboratory tests.
  • Prescription and administration of vaccines pursuant to OHA immunization protocols for pharmacists.
  • Provides consultation to patients or other providers regarding medication use.
  • Provides for the patients health care needs and when needed, arranges care with other qualified professionals.
  • Provides timely follow‑up on patients seen including charting, lab review, and referrals to specialists including providing after hours coverage as required by site.
  • Manages high risk populations in coordination with other providers.
  • Actively supports organizational priorities of patient care, quality, education and sustainability.
  • Collaborates in the development, implementation and standardization and workflows for quality and safety initiatives.
  • Evaluates the appropriateness of medication utilization. Takes action to prevent or correct deficiencies.
  • Develops and conducts educational initiatives for staff.
  • Provides patient and employee education regarding medication use.
