• Assist with intake and documentation of patient history, vital signs, and reason for visit.
• Administer medications and vaccines as ordered.
• Perform point-of-care testing and specimen collection.
• Assist providers with procedures and exams.
• Monitor patient conditions and communicate changes to the care team.
• Ensure proper documentation in the electronic health record (EHR).
• Support patient education and outreach efforts.

• Current and unrestricted LPN license in SC.
• Minimum of 1 year of nursing experience preferred (FQHC or community health experience is a plus).
• BLS certification required.
• Experience with EHR systems (e.g., Epic, Next Gen) preferred.
• Strong communication and teamwork skills.
• Commitment to serving underserved populations.
