Licensed Practical Nurse-Orangeburg

JOB TITLE: Licensed Practical Nurse (LPN)

LOCATION: Orangeburg

• Assesses and monitors patient’s condition; conducts health risk assessments for patients.
• Administers and reconciles medications, treatments and immunizations prescribed by the provider. Calls prescriptions into the pharmacy as directed by the provider. Inquires and documents all OTC medications, supplements and alternative therapies. Provides information on new prescriptions.
• Ensures medication list is up to date in EHS.
• Provides and documents health behavior counseling.
• Prepares patients for various treatments and diagnostic procedures; administers/arranges for various diagnostic tests and treatments as instructed by the provider.
• Obtains specimens for laboratory analysis. Performs tests as appropriate. Notifies patients of labs and radiology results as directed by the provider. Tracks labs and radiology results.
• Coordinates patient panels and gap in care for the clinical team.
• Makes and tracks referrals, including community resources as directed by the provider.
• Maintains accurate and complete patient medical records and other records.
• Provides documented information, instruction and counseling on health maintenance, health promotion, social problems, illness prevention, illness management and medicine use. Visit summaries must be printed and given to patients.
• Follows correct procedures for handling and disposing of biohazardous waste material.
• Complies with all applicable infection control and safety procedures.
• Participates in daily team pre-visit planning (i.e., huddles).
• Orders and stocks supplies for the team.
• Exhibits compliance with FHC’s corporate compliance policy.
• Performs job activities in a respectful, courteous and responsive manner while interacting with patients, visitors and co-workers.
• Performs other duties as apparent or as delegated.

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Current and unrestricted LPN license in South Carolina.
• CPR/BLS certification required.

• Minimum of 1–2 years of clinical nursing experience preferred.
• Experience in primary care, community health, or FQHC setting is a plus.
• Bilingual ability (English/Spanish or other) preferred but not required.
