Overview

The Admission/Discharge Nurse coordinates and facilitates efficient patient admissions and discharges on the telemetry unit. This role ensures smooth transitions of care by providing patient education, verifying documentation, collaborating with the interdisciplinary team, and supporting timely patient throughput. In addition to admission and discharge responsibilities, the nurse will provide coverage for staff lunches and perform leader rounds with patients and families as time permits to support patient experience and safety goals.

  • Job Summary:

    Provide professional nursing care within an assigned unit and coordinate care planning with other disciplines.
  • Work Environment:
    Regularly exposed to infectious, contagious and blood borne diseases. Contact with patients under a wide variety of circumstances. Exposed to unpleasant elements including accidents, injuries and illnesses. Subject to varying and unpredictable situations. Must be able to handle emergency or crisis situations. Is occasionally subject to irregular hours. Occasional exposure to radiation.

  • Financial Responsibilities:
    Financial stewardship of supplies and resources.
Essential Functions
  • Document initial nursing histories, assess patients’ conditions per policy requirements, and develop individual care plans considering physical, psycho-social, environmental, cultural, and spiritual needs.
  • Assist physicians with examinations, procedures, and other processes related to direct patient care per policies and procedures.
  • Evaluate outcomes of patient care, consult with specialists as required, and revise nursing care plans to optimize patient care.
  • Coordinate discharge planning; monitor and record patients’ condition; maintain confidentiality.
  • Use an interdisciplinary approach to formulate teaching plans based on identified learning needs and evaluate learning effectiveness (including family teaching).
  • Coordinate diagnostic and therapeutic services; communicate patients’ condition during hand-offs.
  • Perform other duties as assigned; perform charge duties after six months of nursing experience and required training.
  • R rounds on patients per policy to improve clinical outcomes; orient or mentor new staff; listen effectively to patients to facilitate care requirements; provide first line customer service; follow MUSC Standards of Behaviors; other duties as assigned.
Physical & Mental Requirements
  • Physical requirements:

    stand up to 12 hours; walk, sit, use hands, reach, stoop, kneel, talk, hear, and smell; may lift or exert energy up to 35 pounds; safe patient handling equipment used for transfers greater than 35 pounds.
  • Vision: close, distance, color, peripheral, and depth perception.
  • Mental requirements: read and interpret manuals and procedures; write reports; present information; perform calculations; problem solving; multitask and prioritize responsibilities.
Population Served

This position primarily interacts with all age groups and provides direct patient care. Staff must meet the physical, psycho/social, educational, safety, and related needs of persons in the assigned work area and respond to diverse cultural backgrounds.

Qualifications
  • Graduation from an accredited school of nursing. Bachelor of Nursing degree preferred.
  • Current RN license in the state of South Carolina.
  • Current American Heart Association (AHA) Basic Life Support (BLS) or American Red Cross BLS for Healthcare Providers certification required.
  • Some clinical or hospital experience is preferred but not required.
Physical Requirements
  • Ability to stand or sit for extended periods.
  • Dexterity for tasks such as taking vitals, administering injections, or exams.
  • Capacity to lift/move patients or equipment as needed.
  • Visual and color perception to read medical charts and monitors.
  • Auditory ability to communicate with patients and colleagues and hear alarms or instructions.
  • Stamina to handle physical and emotional demands of patient care and long shifts.
  • Mobility to navigate clinical environments.
  • Compliance with infection control and PPE protocols.
