Job Description
Summary:
Managing patient care at Hollings Cancer Center of Orangeburg and Sickle Cell Clinic of Orangeburg. Responsible for providing social services and coordinating activities to meet patients' social and emotional needs. Identifies and assesses client cases and anticipates their needs related to illness, hospitalization, finances, education and home or community services. Serves as a liaison between health care facilities, community resources and government agencies.
- Interview clients and/or family to assess the client’s developmental level, ego strengths and deficits, situation strengths and weaknesses, and mental status.
- Discuss the proposed plan with the client detailing the recommendations and reasoning, and identify alternative interventions and methods.
- Coordinate external services for clients and serve as client advocate to ensure continuing services are received.
- Meet with community groups to discuss and review available programs and identify areas where resources need to be allocated.
- Develop plans of care through consultation with professionals from areas such as nursing, psychology, psychiatry, medicine and chaplaincy. Plans may include transportation arrangements, home health referrals, hospice referrals, and resources for disparity needs related to care.
- Monitor and document progress of clients and make service adjustments as needed.
- Conduct community planning, support groups, needs assessment and evaluation concerning societal conditions impacting social functioning.
- Education:
Master’s degree in a social work or social welfare program accredited by the Council on Social Work Education. - Credentials: MSW licensure by the South Carolina Board of Social Work Examiners is required.
- Certification in case management is preferred.
