Social Worker, Community Health

This is a full-time, on-site role for a Social Worker based in Orange, CA. The Social Worker will provide support and resources to patients and their families, advocate for clients, and help them navigate healthcare systems and available services. Daily responsibilities include conducting assessments, coordinating care plans, providing counseling, and referring clients to additional support services or community resources. Collaboration with interdisciplinary teams, documentation of client progress, and acting as a mediator in challenging situations are key aspects of this position.

• Strong skills in case management, assessment, and resource coordination
• Proficiency in counseling and crisis intervention techniques
• Excellent communication, teamwork, and interpersonal skills
• Ability to handle sensitive situations with empathy and confidentiality
• Knowledge of healthcare systems, community programs, and relevant regulations
• Experience in creating and executing individualized care plans
• Bachelor's or Master's degree in Social Work or a related field; licensure or certification as required by the state
• Previous experience in healthcare or social services is a plus
