Job SummaryReporting to the Clinic Manager, the Licensed Clinical Social Worker (LCSW) is responsible for psychological assessment; social and practical needs; crisis intervention; consultation, education, community linkages and support services for patients and families coping with an acute or chronic hematological condition.
Scope of ResponsibilityScope of responsibility includes inpatient and outpatient needs assessment, supportive counseling, pain management techniques, emotional coping skills coaching, consultation with medical team in development of viable treatment options to fit patient’s values and means, provide information, education, coordination of care, and advocacy with schools, employers, and/or daycare providers, support groups, and data collection. Additionally, assumes full or partial responsibility for projects designed to support CIBD’s mission;
attends appropriate training, seminars and workshops as required; and, performs related duties as required.
Job Duties
Provides comprehensive psychosocial assessment and provides culturally sensitive and developmentally appropriate services and interventions with patients and families on a timely basis. Advocates for and empowers patients to maintain a level of control over their health and wellness. Documents assessments and interventions in the patient’s medical record in accordance with departmental policy and procedures.
Demonstrates ability to assess adequacy and use of community resources based upon individual needs of the patient and their family; knowledge of and ability to make appropriate referrals to community resources based upon the individual needs of the patients and families; and, coordinates plan of care with community resources such as: schools, Regional Center, Medical Therapy Unit, Social Security, Department of Rehabilitation, and transportation needs, etc.
Demonstrates ability to effectively participate as a member of a multi-disciplinary team. Work with patients, family, and other healthcare team members to develop a comprehensive plan of care that meets the patient and family’s need.
Actively engages in program planning and development; provides and maintains educational materials; and, assists with case management.
Offers psychotherapy to children, families, and adults.
Required Education/Experience/Skills- Master’s in Social Work (MSW) from an accredited institution.
- California Clinical Social Worker License.
- One year of experience in a healthcare setting.
- One year of experience working with children and families.
- One year of experience working with adults.
- Basic Life Support.
- Excellent communication skills (written and verbal).
- Strong Work ethic and knowledge to make appropriate referrals.
- Excellent analytical and research skills.
- Ability to effectively participate as a member/leader of a multidisciplinary team.
- Ability to work with minimum supervision and set priorities.
- Bilingual English/Spanish

Compensation offered will be determined by factors such as location, level, job-related knowledge, skills, and experience.
